(A) Coronal CT image of chest in lung window setting demonstrates large expansile air-filled left hemithorax with no peripheral lung parenchyma and contralateral mediastinal shift, resulting in initial impression of left side tension pneumothorax. However, careful inspection of axial (B) and oblique coronal image (C, along the dotted line in B), reveals clear communication of the left-sided air with the bowel loop/duodenum (arrows in B and C), consistent with diagnosis of tension gastrothorax.
