An elderly male with pathology-proven TCC of urinary bladder grade 2 to 3. At CT: left mild hydronephrosis is seen with soft tissue encircling aorta; the soft tissue thickening is seen posterior to aorta and at same location mild aortic aneurismal dilation is noted. Presence of TCC of urinary bladder favors retroperitoneal lymphadenopathy; presence of aortic aneurysm favors retroperitoneal fibrosis. Retroperitoneal fibrosis is usually mildly enhancing with no mass effect upon aorta. In lymphoma/lymph node metastasis usually no enhancement and aorta is slightly displaced anteriorly. Retroperitoneal fibrosis usually encircling entire aorta.
