Radiology boards wisdom of the day:
The PTT is the largest of the three medial flexor tendons, and the most likely (of the three) to be abnormal. It provides a significant amount of support to the arch of the foot, and therefore tears often lead to flatfoot. PTT Tears associated with Middle-aged (or older) women, RA patients; accessory navicular bones; sinus tarsi syndrome. A small spur or periostial reaction along the posterior aspect of the medial malleolus is a secondary sign of PTT tendon rupture. Spastic (painful) Flatfoot is associated with a tarsal coalition.
